A hard fall

Yesterday, after a very busy medical guard, we received a particular case in the emergency room:

It was the case of a 9 year old girl who was playing around an area under construction with her friends, with a lot of dangerous obstacles the children ran as they all do in their childhood full of games and fantasies. Probably those things that we see as dangerous for them represent challenges and curiosities.

After falling from a height of approximately 1 meter to the edge of sharp metal structures, Julia slides very close and falls abruptly on her back. The movement was so rapid that he did not notice the moment when his arm rosed over her inner part near her chest.

Her friends immediately realized the gravity of the matter when they saw a large amount of blood on the ground as she screamed desperately for help.

Una dura caída

El día de ayer luego de una guardia medica bastante ajetreada nos llegó un particular caso a la sala de emergencias:

Se trataba del caso de una niña de 9 años que se encontraba jugando alrededor de una zona en construcción con sus amigos, con una gran cantidad de obstáculos peligrosos los niños corrían como todos lo hacen en su infancia llena de juegos y fantasías. Probablemente aquellas cosas que nosotros vemos como peligrosas para ellos representan retos y curiosidades.

Luego de caerse de una altura de 1 metro aproximadamente al borde unas estructuras metálicas afiladas, Julia resbala muy cerca y de manera abrupta cae sobre su espalda. El movimiento fue tan rápido que no se percató del momento en que su brazo rosó sobre su parte interna cercana al tórax.

Sus amigos de inmediato se percataron de la gravedad del asunto al ver una gran cantidad de sangre en el suelo mientras ella gritaba desesperadamente pidiendo ayuda.

The Arm

The arm represents anatomically the region between the shoulder and elbow joint, and is known to surgeons as a transition area for the great vessels and nerves that run in the direction of the hand.

It is divided into two well known large muscle compartments, the muscles for elbow flexion (mainly the biceps muscle) and the muscles for elbow extension: the triceps. In turn surrounded by large arteries, nerves and veins.

The arm muscles are of great importance for daily activities, even in gyms you are given adequate importance for lifting almost any exercise with weights. They are powerful and are composed of several muscle fascicles, so the injury of any of them generates important limitations.

El Brazo

El brazo representa anatómicamente la región que se ubica entre la articulación del hombro y del codo, de alguna forma es conocida entre los cirujanos como una zona de transición de los grandes vasos y nervios que van con dirección hacia la mano.

Esta dividido en dos grandes compartimientos musculares bien conocidos, los músculos para la flexión del codo (principalmente el músculo bíceps) y los músculos para la extensión del codo: El tríceps. A su vez rodeados de grandes arterias, nervios y venas.

Los músculos del brazo son de gran importancia para las actividades a diario, incluso en los gimnasios se le da la importancia adecuada para el levantamiento de casi cualquier ejercicio con pesas. Son potentes y están compuestos de varios fascículos musculares, por lo que la lesión de cualquiera de ellos genera limitaciones importantes.

Repair in the operating room

Julia went to the hospital after that serious incident with her parents, and after the evaluation of our medical team we found a large semicircular wound around her arm covered with abundant blood.

We immediately offered her enough painkillers to ease the great pain she was in and put in plenty of saline in view of the large amount of blood lost. It is always important to prevent patients from falling into hypovolemic shock from blood loss, as this causes heart attacks.

After this, we went to talk to her parents to inform them that in view of the seriousness of the situation, a surgical examination procedure had to be performed, where she was anesthetized.

We immediately entered Julia into the ward where after she was sleeping we were able to clean the area and do a thorough wash, we could see that the wound was very extensive and had severed part of the muscles.

We identified the triceps muscle of the partially injured arm and proceeded to suture its ends and repair it. It's really laborious because we have to avoid damaging other nearby arteries.

In the end, the surgery was a success after an hour of anesthesia. So after learning a valuable lesson, this little girl will think twice before doing another action like this.

Reparación en el quirófano

Julia acudió al Hospital luego de aquel grave incidente junto a sus padres, y luego de la evaluación de nuestro equipo médico evidenciamos una gran herida semicircular alrededor de su brazo cubierto de abundante sangre.

De inmediato le ofrecimos analgésicos suficientes para calmar el gran dolor que tenía ella y le colocamos abundante suero en vista de la gran cantidad de sangre perdida. Es importante siempre evitar que los pacientes caigan en shock hipovolémico por la perdida de sangre, ya que esto causa ataques al corazón.

Luego de esto nos dirigimos a hablar con los padres de ella para informarles que en vista de lo grave de la situación había que realizar un procedimiento quirúrgico de exploración, donde estuviera ella anestesiada.

Inmediatamente ingresamos a Julia a la sala de pabellón donde luego de que ella estuviese durmiendo pudimos limpiar la zona y realizar un minucioso lavado, nos pudimos dar cuenta de que la herida era muy extensa y había seccionado parte de los músculos.

Identificamos el musculo tríceps del brazo parcialmente lesionado por lo que procedimos a realizar la sutura de sus extremos y repararlo. Es realmente laborioso ya que tenemos que evitar lesionar otras arterias cercanas.

Al final la cirugía fue todo un éxito tras una hora de anestesia. Así que luego de haber aprendido una valiosa lección, esta pequeña lo pensará dos veces antes de realizar otra acción como esta.
